Physical Therapist- PT Home Health Job // Salem, OR

New grads welcome!Looking for Full-Time, Part-time, and Contract Physical Therapists to provide home care to patients when skilled health care in their home means independence, comfort and the possibility of a faster recovery.
As a Physical Therapist for our client's Home Health division, you will be responsible for clinical assessments, plan of care development and case management. You must also have strong written and oral communication with interpersonal skills. Team collaboration and clinical reasoning are also necessary.
Responsibilities:
The PHYSICAL THERAPIST is a qualified health care professional who directs, supervises, evaluates, and provides physical therapy services to clients in the home as prescribed by the attending physician.
He/she is responsible for: (1) providing skilled physical therapy care to patients with impairments, functional limitations, disabilities, or changes in physical function resulting from injury, disease, or other causes; (2) recording clinical and progress notes; and (3) coordinating other patient care activities/services to provide the highest quality of patient care in the geographic service area to which he/she is assigned.
Qualifications:
1. Graduation from a physical therapy school accredited by the Commission on Accreditation in Physical Therapy Education.
2. Licensed or registered as a Therapist in the state(s) in which he/she is required to practice.
3. A minimum of one year of experience in acute, rehab, outpatient, or long term care setting required.
4. Ability to relate to and treat acutely ill/chronically ill patients/families and their particular problems.
5. Must possess strong patient assessment, patient management, and documentation skills.
6. Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the patient.
7. Must possess the disposition and personality to work harmoniously with a wide variety of people and all levels of staff, and interact professionally as a member of a multi-disciplinary team.
8. Must possess a strong commitment to the goals, mission, and philosophy of the organization.
9. Ability to work flexible hours as required to meet identified patient needs.
10. Absolute integrity and the use of good judgment to handle the professional and confidential nature of assignments..
11. Ability to adapt to changing organizational needs.
This position requires a Doctorate of Science in Physical Therapy and current PT licensure. A minimum of one year experience in a home care setting would be preferred
